Supported Living Partners is a Medicaid Provider Agency which provides attendant care services to Individuals with Intellectual or Developmental Disabilities (IDD) as defined in Oregon Administrative Rules.  To qualify for services, adults with IDD, or their parent or legal guardian must contact their service coordinator in the county in which they live and let them know you would be interested in using Supported Living Partners for their attendant care needs.  Referral may also be made through the Oregon Developmental Disabilities Division (ODDS).
