How to Become a Client

Supported Living Partners is a Medicaid Provider Agency which provides attendant care services to Individuals with Intellectual or Developmental Disabilities (IDD) as defined in Oregon Administrative Rules.  To qualify for services, adults with IDD, or their parent or legal guardian must contact their service coordinator in the county in which they live and let them know you would be interested in using Supported Living Partners for their attendant care needs.  Referral may also be made through the Oregon Developmental Disabilities Division (ODDS). name, level of experience and placement needs, and the 
earliest date you could be available to start.

For persons with IDD, you must be referred by your county

All potential staff must be interviewed, their references must 
be checked, and must undergo a background check prior to 
receiving an offer of employment. Much of the interview process is possible to be completed via zoom.
